Building/Facilities:

  • Facility Use Request: for use of FUMC facilities/rentals. Paper copies can be found in the main office.
  • Board of Trustees request form: for maintenance and building needs. Paper copies (bright orange) can be found on the boards outside the workroom and past the sanctuary entrance.
  • Endowment Funds Request: Groups, committees and individuals identify needs which Endowment could help fulfill in the coming year. These needs can require project funding for one year or for multiple years. Requests are evaluated by the committee in keeping with our charter and mission.

Children/Youth

  • Release forms can be found on the children and youth ministry pages.

Employees:

  • Safe sanctuaries documents can be found on the employment page.

Miscellaneous:

  • New member information form: scroll to the bottom of our homepage, linked under “new member classes.”
  • All forms for weddings, baptisms, and funerals can be found on the linked page.
  • Incident report form: for accidents/injuries on church property. Paper copies (bright orange) can be found on the boards outside the workroom and past the sanctuary entrance.
